Remote Cardiology Patient Navigator (RN/MA/CNA)

About the role

Our client is looking for a Remote Patient Care Coordinator, also known as a Cardiology Patient Navigator, to join a busy US‑based cardiology team. The position focuses on patient communication, scheduling, care coordination, and clinical documentation for a high‑volume caseload of 150+ patients.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR systems.
• Conduct telephonic or remote outreach to reinforce care instructions and coordinate next steps.
• Support patients navigating cardiology pathways, including CHF management protocols.
• Track testing schedules, appointments, and remote monitoring workflows.
• Coordinate with the clinical team to ensure continuity of care and escalate clinical concerns when needed.
• Maintain accurate clinical documentation and ensure HIPAA compliance.
• Assist with billing documentation and insurance coordination as required.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• Minimum 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or management, preferably in a remote or telephonic environment.
• Strong communication skills and ability to handle a high‑volume, remote workload.

Required skills

• Proficiency with EMR/EHR systems.
• Familiarity with remote patient monitoring platforms.

What we offer

• Full‑time remote position with flexible US Central Time business hours.
• Hourly compensation ranging from $5 to $7 USD.
