Remote Cardiology Patient Navigator – Patient Care Coordinator

About the role

Our client is looking for a Remote Cardiology Patient Navigator, also called a Remote Patient Care Coordinator, to join a busy US‑based cardiology team. Working from anywhere, you will support patients through scheduling, communication, and care coordination while ensuring accurate clinical documentation.

Key responsibilities

• Manage patient scheduling, reminders and follow‑up workflows in EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions, support CHF management protocols and guide patients through cardiology pathways.
• Track testing schedules, remote monitoring data and coordinate appointments for a caseload of 150+ patients.
• Escalate clinical concerns to providers within scope and maintain HIPAA‑compliant documentation.
• Assist with billing documentation and insurance coordination as needed.

Required profile

• Patient‑first attitude with strong phone communication skills.
• Ability to stay organized under high volume and pressure.
• Self‑directed and comfortable working remotely with evolving processes.
• Collaborative, outcomes‑focused and proactive in communicating with providers and patients.

Required skills

• Clinical credential (CNA, MA, RN or equivalent).
• 3+ years experience in healthcare, telehealth or medical office settings.
• Experience in care coordination or care management.
• Proficiency with EMR/EHR systems.
• Familiarity with remote patient monitoring platforms.
